Nagoya Child Medical Subsidy: Medical Certificate and Care Outside Aichi

This is a municipal medical expense subsidy, so it assumes the child already has health insurance coverage; Nagoya City reduces the remaining insured copayment on top of that coverage. Eligible children are those who live in Nagoya City and are enrolled in health insurance, from birth through the end of the fiscal year in which they turn 18. Depending on the child's age, either the guardian or the insured person applies. There is no income limit, but children eligible for disability medical assistance or single-parent family medical assistance, those receiving livelihood protection, and those in a child welfare facility are not eligible for this subsidy. Points to note

• Some assume that because Nagoya City offers resident-registration guidance for foreign residents, foreign families automatically qualify. • Presenting the medical certificate does not make every cost free. Bed-fee differences, hospital meals, health checkups, vaccinations, and document fees are excluded even at Aichi Prefecture providers.
• Care received outside Aichi Prefecture, or cases where the certificate cannot be shown, is not automatically covered at the counter. These are handled as reimbursement after review, and the reimbursement deadlines to the city and to health insurance differ.

The copayment is only treated as free when the required insurance proof and the medical certificate are presented together at a provider within Aichi Prefecture; bed-fee differences, hospital meals, health checkups, vaccinations, and document fees remain excluded regardless. If care was received outside Aichi Prefecture, or the certificate could not be presented, the cost may still qualify for reimbursement after Nagoya City reviews the claim -- but the deadline to claim from the city is five years from the day after payment, while a separate health-insurance reimbursement claim, where required, must be filed within two years.
